Caring Hands, a program with Advocate Hospice

At Caring Hands, we focus on compassionate care that provides dignity and comfort to patients and their families when they need it most. Pain management and spiritual support are always part of the process.

Our interdisciplinary team of professionals provides input and overall care to hospice patients. They work together with the patient and family to meet specific quality of life needs. The team includes:

• Patient Physician–the patient’s trusted personal care provider.
• Hospice Physician–for pain and symptom management.
• Medical Director–provides team leadership
• Licensed Nurses–for daily, ongoing care
• Home Care Aides–for personal care and daily activities
• Social Workers and Grief Counselor–provide supportive services
• Pastoral Care–offers spiritual counseling and advisement
• Volunteers–for assisting patients and families with personal needs and requests

Other hospice services offered:

• Music therapy
• Pet therapy
