Download our gift form, which can be completed and sent with your check (payable to Lutheran Life Communities Foundation) or credit card information to Lutheran Life Communities Foundation, 3150 Salt Creek Ln., Arlington Heights, IL 60005. You can also drop off your gift and form at the Foundation office at the Lutheran Life Communities Ministry Center, 3150 Salt Creek Ln., Arlington Heights, IL.

If you prefer, you can give your credit card information to one of our staff over the phone by calling the Foundation office at (847) 368-7371.

Make a gift of stock or mutual fund shares.

You can make a gift of appreciated stock or mutual fund shares and bypass the capital gains tax. Please contact the Foundation office at (847) 368-7371 for instructions on how to make a stock or mutual fund transfer.

Gifts accepted by the Foundation that are restricted by the donor for a particular campus or program are used only for the designated purpose and will be held in separate fund accounts within the Foundation.
