Assuring the correct compliance with an individual’s daily medication schedule is an essential factor in quality of care. @HomeMatters provides a specific program that provides this important medication management service. The medication Management service provides the following:
Professional nurses will:
- Notify the physician of the @HomeMatters medication management service
- Verify medication regimen for accuracy of medication, dosage, route and time schedule
- Set-up medications every two weeks in the appropriate container
- Request medication refills, as indicated, from the local pharmacy or mail-order resource
The medications are organized in such a way that the appropriate medications are dispensed in designated containers, i.e. cassettes or a medication dispensing machine. The stock medications that have not as yet been dispensed are secured in a locked medication bag for security and organizational purposes.
If adjustments or additions are needed the nurse will be notified by the physician or client representative to make the appropriate changes to the medication regimen.

- Assisting you to accurately set up your medications with proper dosing and timing in a simple and easy to understand manner.
- Organize so you are well prepared to know when to refill or stop the prescribed medications
- Help you fully understand what your medications are and how they should be taken to ensure optimum effect.
We can provide our nurse to visit with you once, or as often as you deem necessary to help keep you well and safe at home.
